Please call our office to schedule your first appointment or send us a request for an appointment by registering on the patient portal. We will need some basic information from you to schedule your first appointment. Note for many insurance plans we are not currently accepting new patients.
You can view of most of the insurance plans we accept by visiting Rush Health. If you do not see your insurance carrier or plan listed, please contact our office for more information regarding other plans we may accept. Please note that for many insurance plans we currently only are caring for existing patients.
Ask your pharmacy to submit the refill request directly to our office. We will review the request and return it to the pharmacy when completed. You can also request refills on the patient portal.
