General Information
You can easily review or pay your statements by logging in to the patient portal. If you have questions regarding which insurance plans our practice participates in, please contact our office.
You will be responsible for any services not covered under your insurance contract or for services rejected by your insurance carrier. Patients are also responsible for any copays, coinsurances or deductible amounts.

Co-Insurance/Co-Pay
If you insurance requires that you will be responsible for part of a bill, this co-insurance must be paid prior to the services being provided. Co-pays are to be paid at the time service is provided. It is required by your insurance carrier that we collect copays at that time.
